There are three ways to submit your Win/Loss Statement Request to Bear River Casino Resort:1. Drop off: Visit Casino Services and submit your completed Gaming Activity Request Form.

2. By Mail: Please download and print the Gaming Activity Request form. After completing the form, please mail it to the following address:

3. By Email: Please email your completed form to biancalobos@bearrivercasino.com. *Please send a copy of your ID/DL.
